If physicians have received hospital credentialing through Medical Provider Resources, they do not need to complete a membership application. They only need to email their name and their practice/group’s name, address and phone number to membership coordinator Rhonda Welch at rhondawelch@med-soc.org. Physicians can also check the MSSC membership box on the MPR credentialing application form.
